Landing It

Priya sends Maren a screenshot of an empty queue. The recovery Maren designed meets the firm's habit of moving work before the work is ready.

At 8:14, Priya sent Maren a screenshot of an empty queue. No message, just the screen. Maren looked at it for several seconds before calling, and Priya answered on the first ring.

"What happened?"

"They moved my live work."

"Who?"

"My manager."

"Why?"

"There’s a review." A pause. "The workflow flagged my interventions. The cases I changed that the agent had already cleared."

Maren was already standing. "Since when?"

"The flag’s been open a while. He acted on it this morning."

"Where are you?"

"At my desk."

"The meeting starts in sixteen minutes. Are you coming?"

Another pause, longer than the first. "Yes."

Maren closed her laptop.

Priya was still at her desk when Maren reached the transition floor. The empty queue sat open on one screen, and on the other was a document library, no cases, no deadlines, no names. Priya looked up as Maren pulled the chair from the next desk.

"They didn't take away my access," Priya said. "I can review documentation."

"For how long?"

"They didn't say."

"Did they say this was disciplinary?"

"They said it wasn't."

"How many times?"

Priya almost smiled. "Twice."

"Did they mention the meeting? Your survey comment? The review I circulated?"

"None of it. They said operations had questions about undocumented interventions."

Maren looked at the blank queue. "You documented the changes you made in the cases."

"Yes."

"But not the decision to open them."

"No."

"Because there was no field for that."

Priya looked at her for a moment. "There was no decision. I saw something. I opened it."

"That's a decision."

"Not one the process knew about."

Maren checked the time. Eight twenty-one. "I'm sorry," she said. "I used the comment. I put your name on it. And now this."

"I told you to. Twice." Priya closed the document library. "And I was doing this before you got here." She picked up her notebook. "Remember, don't make me regret giving you permission because you regret asking. The meeting's in nine minutes."

Gerald was already in the room when they came in, Ruth and Daniel with him, Tomas arriving behind Maren. Desmond came in last and stopped when he saw Priya, his eyes moving to the notebook in her hand, then to Maren.

"What happened?" he asked.

Priya sat. "My queue is being reviewed."

"That's not what I asked."

"My live work's been reassigned."

Desmond took out his phone, and Priya said, "No," before he had it unlocked.

"No what?"

"Whatever you're about to do."

"I have options."

"I know."

Gerald had been watching them. "What happened?" he said, looking between Maren and Priya.

Priya turned toward him.

Gerald extended his hand. "You must be Priya."

"I am. Nice to meet you."

"Gerald Voss. Likewise."

Maren said, "Her manager flagged interventions she made outside the standard flow. Her live work was reassigned this morning."

Gerald sat and turned to Maren. "Is this connected to your finding?"

"It may be."

"How?"

"The audit and the finding are looking at the same cases. The finding includes twelve that looked routine until someone read them closely enough to find they weren't. Priya handled most of them. The audit sees the same interventions as a process problem."

Gerald looked back at her. "You changed cases outside the standard workflow?"

Priya answered before Maren could. "I opened cases the workflow didn't tell me to open."

"That's not exactly what I asked."

"No, it’s not." She waited a beat. "Sometimes I changed them."

"How often?"

"I don't know."

"Why?"

"Because they needed changing."

Gerald's expression stayed calm, which was its own kind of pressure. "How did you determine that?"

"I read the file."

No one spoke. Maren put her folder on the table, and Gerald looked at it, and then at her. "Should we begin?"

"Yes."

Maren had built a deck. She didn't open it. Instead she laid five pages in the middle of the table: the manual-touch trend, the project closeout timeline, Priya's raw survey comment, Marcus Bell's case record, and Ruth's business-case dependency.

"No slides?" Gerald said.

"Not yet."

"That's ominous."

"Just slightly."

Ruth pulled the manual-touch trend closer, and Gerald told Maren to walk him through it.

"Specialist intervention fell after each expansion," Maren said, pointing to the first page.

"Yes."

"The business case expected it to fall."

"Yes," Ruth said.

"It fell farther than the target. The capacity gain depended in part on that reduction."

"Yes."

"We know this," Gerald said.

"We do." Maren moved to the second page. "During rollout, the firm also had weekly case review, office hours, specialist feedback, adoption pulses, and cross-functional implementation review. Each one ended during or shortly after closeout."

Daniel nodded. "As planned."

"As planned," Maren agreed, and moved to the third page, where Priya's comment sat above the summary it had become. She read the first aloud, then the second.

Some cases look clean in the system but need a second read before the first message goes out.
Isolated requests for additional exception guidance can be addressed through existing escalation pathways and future template optimization.

Gerald read both, then looked at Priya. "Did you write this?"

"Yes."

"What did you mean?"

"I didn't know at the time," Priya said. "That was part of the problem."

"What did you know?"

"That sometimes a case looked fine, and I opened it anyway."

"Why?"

"I don't know, specifically. Usually, I found the reason after I opened it, not before." She folded her hands. "Sometimes the dates were strange. Sometimes a note didn't match the category. Sometimes there was nothing I could point to."

"Then how could anyone build a process around that?" Gerald said.

"I wasn't asking for a process."

"What were you asking for?"

Priya looked at the survey comment again. "How to know. How to know when a clean case needed opening." She pointed to the summary. "I wasn't asking when to escalate. Escalation means you’ve already seen something. I was asking about before that. How to know before there was something to escalate."

Maren watched Gerald read the sentence again, the coded theme and the disposition, exception guidance, and existing escalation pathway, and future template optimization, and something in his face slowed.

"All right," he said. Then, to Maren: "What happened to the comment?"

"It was coded correctly, under the categories available."

Daniel's head came up. "Correctly?"

"The team had a category for exception guidance," Maren said. "A category for template issues. A category for escalation. Which one was this?"

Daniel looked at the raw comment for a long moment. "None of them."

"So, it went to the closest one."

Daniel said nothing, which was the answer.

Maren moved to the fourth page, Marcus Bell's record, closed and on time, no flag, no escalation, no specialist intervention, no survey response. "Which existing channel would have found him?" she said, and when Gerald didn't answer, she turned to Tomas. "Agent flag?"

"No."

"Specialist escalation?"

"No."

"Employee complaint?"

"Not through the firm."

"Quality review?"

"He was eligible for random sampling," Tomas said.

"Was he sampled?"

"No."

Maren looked back at Gerald. "The current controls could have found him. They didn't. What told the firm he wasn't a clean case?"

Gerald looked at the record. "The post."

"Yes." Maren sat down, and nobody spoke for several seconds.

Then Gerald said, "That establishes a miss. It doesn't establish a pattern. It doesn't establish quality erosion. It doesn't establish how many other cases there are."

"You’re right." Maren said.

"Then what exactly are you asking me to accept?"

She looked at the five pages. "That the firm can't answer the last question." A beat. "Neither can I."

Ruth opened her laptop. "I want to be precise before this gets less precise."

"My favorite kind of warning," Desmond said.

Ruth ignored him. "The financial case remains valid. The savings occurred. Cycle time fell. Capacity increased. Specialist intervention fell, and that decrease was part of the benefit design." She looked at Gerald. "Nothing in Maren's findings changes those facts."

"I agree." Maren said.

"You're about to add something."

"I am."

"Please make it a different sentence."

Maren almost smiled, and stood, and went to the whiteboard. At the top she wrote CAPACITY GAIN, and under it FEWER SPECIALIST TOUCHES.

"Fair?" she asked Ruth.

"Yes."

Below that: MORE STANDARD CASES PROCEED WITHOUT HUMAN REVIEW. Ruth said nothing. Maren looked at Tomas. "Fair?"

"Yes."

Below that: THE CLASSIFICATION MUST BE RIGHT.

Gerald leaned forward. "There. That's the issue. Marcus was classified as standard when he shouldn't have been. So, we improve classification."

"How?"

"Rules. Data. Case criteria." He looked at Tomas.

"We'd need to understand what was missed," Tomas said.

"We investigate the case, find the classification gap, fix it," Gerald said, turning back to Maren.

"And Denise Harmon?"

"Same thing."

"The other eleven?"

"Same thing."

Maren looked at Priya, and Priya looked back, and then Maren said, "What if there's no common rule?"

Gerald's expression changed. "Then we find a different control."

"What kind?"

"I thought that was what you were here to tell me." The words weren't cruel, which was what made them land. Maren looked at the board.

"I can tell you what I'd do now," she said. "Pause expansion. Restore a human sample of cases the system considers clean."

"How large?"

"I don't know yet."

Gerald looked at Ruth, who said, "The size matters."

"I know." Maren kept going. "Review the twelve intercepted cases and Marcus against the same question. Not what rule was violated. What did the human notice that the system did not?" Tomas was writing now. She added a line to the board: WHO CAN STOP A CLEAN CASE?

"The specialist," Gerald said.

"Can they?"

"Yes."

"Not really," Priya said.

Gerald turned. "You just said you did."

"I said I do it. That's not the same." She was quiet a moment, choosing it. "I can open the case. I can change the message. I can place a hold on it. But the process says I shouldn't need to do any of that unless there's an exception."

"So, if you do it anyway?"

Priya glanced at the notebook. "Apparently, my queue disappears."

"I had the ability." She looked at the whiteboard. "I didn't have the authority."

Maren didn't write anything else. She didn't need to.

Gerald stood and walked to the window, and no one spoke until he turned back. He looked at the board for a while, the four lines Maren had written, and then at the five pages on the table.

"Tell me what actually happened," he said. "Not the risk. What happened."

Maren stayed where she was. "The benefit was built by removing the human second read. That was the design. Fewer touches, more capacity, and it worked. The numbers are real." She looked at the board. "But the second read was doing two things. It was slow, and it was catching. We measured the slow. We never measured the catching, so when it went away, only half of it showed up as a change. The half that showed up was the capacity. The half that didn't was Marcus."

"We closed the other channels on purpose," Daniel said. "The review meetings, the pulses. Those were implementation scaffolding. You take scaffolding down when the building stands."

"You do," Maren said. "And those were the only places, while they lasted, where someone was comparing what the agent decided with what an experienced person would have decided. Not as a control. Nobody called them that. But that’s what they were doing, and nothing replaced them when they came down."

Gerald came back to the table. "So, the finding is that we took down something we didn’t know was load-bearing."

"Yes."

"Because it was never written down as load-bearing."

"It was never written down at all." Maren looked at Priya. "It lived in a habit. In a second read a specialist did because she always had. You can’t decommission a thing like that on purpose, because nobody ever commissioned it. It just stops."

Gerald was quiet for a moment. "That isn’t a comfortable finding to take upstairs."

"I understand."

"It says we ran a successful implementation and removed a safeguard we couldn’t see, and we won’t know what it cost until the cases surface on their own." He said it flatly, working out how it would sound in a room three floors up. "The board is going to ask me how many Marcus cases there are."

"I know."

"And I’ll have to tell them the honest answer is that the current operation can’t produce that number. Which is the finding." He looked at her. "You understand what that does to me in that room."

"Yes." Maren did not soften it. "The firm reported a benefit and a clean record, and the honest version says the record was clean because the thing that would have found the problem was gone. That lands on the sponsor. It lands on you, unfortunately."

Gerald absorbed that without flinching, which was its own kind of weight. "Then let’s be sure it’s true before I carry it." He sat. "Walk me through the fix. All of it. What’s broken now, what we do about the cases already in the system, and what has to be different before this goes anywhere near another function."

Maren wrote a new heading on the board, under the diagnosis. WHAT IS BROKEN NOW.

"Two things, and they’re different problems. The first is the cases already in the system. Every standard case the agent has closed since the second read came down went out without the thing that used to catch Marcus. Most of them were fine. Some of them weren’t, and the firm has no record of which is which, because a case that needed a second read and didn’t get one looks identical to a case that never needed one."

"So, we read them," Gerald said. "The ones already closed."

"A sample first, to learn what we’re looking for. Then, as many as the sample says we have to."

Ruth had opened her laptop. "How many cases are we talking about, in total, since the second read came down?"

Tomas checked. "Standard involuntary transitions after the second expansion. Two thousand one hundred eighty-four."

The number sat in the room.

"We aren’t reviewing two thousand cases on a hunch," Gerald said, but he said it as a question.

"You aren’t reviewing them on a hunch," Maren said. "You’re reviewing them because one of them became the thing people are sending each other about this firm, and you found out from the internet instead of from your own controls. The review is how you find the next one before he finds a parking lot."

Ruth was working. "A standing human sample of clean cases has a cost. It gives back part of the capacity gain, because the capacity gain came from not doing it." She looked up. "I can price the review. I can price the restored sampling. What I can’t price is the risk of the cases we don’t review, because I have no number for how many of them are Marcus. Nobody does. That’s the same hole, seen from finance."

"You’re finance," Gerald said. "You always have a number."

"I have a number for things that have entered a financial system." Ruth's voice did not change. "Marcus entered a financial system the day he closed clean and on time. He just entered it as a success."

Nobody spoke for a moment.

Daniel had been quiet, and then he said, "I have a question for you, Priya." He turned to her. "When you opened those cases, the ones nobody told you to open, what were you doing?"

Priya frowned. "Reading them."

"I know. Were you validating the agent?"

"No."

"Checking quality?"

"No."

"Looking for exceptions?"

"No."

"Then what?"

Priya thought about it. "My job." Daniel leaned back, and nobody spoke, and Priya looked uncomfortable with the attention but kept going. "Before the agent, the case arrived, and I read it. Then the agent arrived. And after a while, the case arrived already done." She paused. "I stopped being the person who decided what kind of case it was. The system had already done that part."

"You could override," Daniel said.

"Yes."

"Then you still had the decision."

"No. Because override means the first decision already happened." Daniel did not answer, and Priya looked toward Gerald. "I didn't know how to say that before."

Gerald's face had gone very still.

"I liked the agent," Priya said.

"I know."

"No." The word was quiet. "You know the scores." Gerald said nothing. "I liked it. I still do. Most of the cases are routine. They shouldn't take half a day."

"That was the point," Gerald said.

"Yes."

"And the specialists were meant to spend more time on the hard cases."

"Correct. Then what happened is the hard cases didn't always arrive looking hard."

Gerald looked at Marcus Bell's record, and for the first time that morning, no one in the room spoke to help him.

Gerald turned to the board. "That’s the current operation. You said there were two problems."

"The second is scale." Maren wrote it up. BEFORE THIS GOES ANYWHERE ELSE. "You have the model queued for three more functions. The instinct is to fix the four holes we found here and roll it out with the fixes attached. Better classification. A restored sample. A rule for when a specialist stops a case." She looked at the board, at WHO CAN STOP A CLEAN CASE. "And you should do all of that. But if that’s all you do, you’ll build the same failure somewhere I can’t predict, because the four holes aren’t the disease. They’re where the disease surfaced here."

"Then what’s the disease?" Gerald said.

"You ran this like a tool rollout. You put in software, you trained people to use it, you measured whether they used it, and they did, and you called it done." She paused. "But it doesn’t behave like a tool. A tool waits for a person to decide. This one decides, and the person is left to catch it if they can. That’s a different kind of thing in the workflow, and it needed a different kind of question at the start, and it never got one." She tapped the board. "Nobody ever wrote down who’s allowed to stop a clean case before it goes out. Priya could, technically, and her queue vanished when she did. So, the real answer, the one the system enforced, is that nobody was. The agent decided, and silence counted as agreement, and a man got congratulated on his own layoff because no person was ever named as the one who had to say yes before it went."

Gerald looked at the line on the board for a long moment. "Before we scale, we decide that. For each step. Who has to say yes, and what happens if they say nothing."

"For each step. Before it launches, not after it breaks." Maren capped the marker. "That’s most of what would have caught this. Say who’s accountable when the agent decides. Measure the catching, not just the using. Keep one live channel that compares the agent against a person, permanently, not as scaffolding. Restore the sample. If you build those into the next three the way they were never built into this one, you close what we can see."

"What we can see," Gerald repeated.

"Yes." And this was the part she had not planned to say, and said anyway. "That’s the part I keep landing on. Everything I have given you today is built out of Marcus. I know what to look for because he already happened. The sample, the accountability, the channel, all of it designed to catch the next case that fails the way this one failed."

"And if the next one fails a different way."

"Then I don’t know that we catch it." The room was quiet. "This isn’t a fixed thing you’re installing. It’s going to keep changing after you launch it. The use cases will widen. What counts as a good message this quarter won’t be what counts next quarter. The agent gets better, and it gets used for more, and every time it does, there’s a new way for a case to be wrong that nobody has seen yet, because it hasn’t happened yet." She looked at the board, all of it, the diagnosis and the fixes. "I can build you the instrument for the failure that already happened. I don’t have one for the failure that hasn’t. I’m not sure anyone does. And a system like this produces a new one of those every time it changes."

Gerald did not answer right away. Then he said, "That’s the actual finding, isn't it. Not Marcus. That."

"Marcus is how you know it’s true," Maren said. "But yes. That."

Gerald sat with it. Nobody in the room filled the silence for him, and after a while, he nodded, once, the way a person does when they have stopped looking for the version of a thing that costs less.

"All right," he said. "Here’s what happens. I’m pulling the scale packet. All three functions, on hold, until we have done the review and built what you just described into the design." He looked at Tomas. "We restore a human sample of clean cases this week. Size to follow, and Ruth prices it before I take anything to the board." He looked at Daniel. "We stand up the case review. Every standard transition since the second expansion, sampled first, and we go where the sample tells us." He looked at Maren. "And the finding goes to the board as you have it. Not the softened version. The one that says the current operation can’t tell us whether it’s holding, and here’s why, and here’s what we’re doing about it."

"That’s a hard thing to walk into a board with," Maren said.

"It’s a harder thing to explain after the second Marcus." He almost smiled. "I’d rather be the one who brought them the bad number than the one who didn’t have it when they asked."

Ruth closed her laptop. "I’ll have the cost of the review by Thursday. The cost of not doing it I’ll keep flagging as unpriceable, in writing, so it’s on the record that we chose to spend against a risk we couldn’t size."

"Put it in exactly that language," Gerald said.

He turned to Priya last. "Your manager pulled your live work this morning over an audit flag. I’m not going to reach down and reverse a line decision from in here. But the thing that audit’s treating as a defect is the same thing this whole review exists to protect, and I’m going to make sure the people running the review know that before they touch a single one of your cases."

Priya looked at him. "Thank you."

The engagement had three days left on the original contract, and Maren spent them the ordinary way, writing it up, handing off, sitting with Desmond over the parts of the review that would outlast her. Gerald did not offer to extend her, and she did not ask him to. The recovery would be internal, led by people who had to live inside it, which was the right way for it to go. On the last afternoon, he came down to her office himself, which surprised her, shook her hand, and said the finding had been correct, that he would not enjoy Thursday, and that those two things were allowed to both be true. Then he said the thing she would think about later, on the train.

"When we have built the part that catches what we can see," he said, "I may call you about the other part. The one you said nobody has yet." He did not make it a promise. "If you figure out what that instrument looks like, I’d like to be early to it."

"So would I," Maren said, and meant it more than he knew.

Priya caught her by the elevators, still carrying the notebook.

"You have to stop just appearing places," Maren said.

"I was here first." Priya almost smiled. "They found eleven more cases I changed after full rollout. The audit did." The doors opened; neither of them moved, and the doors closed again. "Some of them shouldn’t have been changed. Some should."

"How many of each?"

"They don't know yet. That’s what the review is for now." She looked at the notebook. "I stay off live work until it’s done."

"How long?"

"They don't know that either." The doors opened again. Priya did not get in. "That seems to be going around."

"It does." Maren held the door. "For what it’s worth, the thing that pulled your queue and the thing that’s about to clear it are the same review. It just has to look at you as a problem for a while before it can see you as the reason it exists."

Priya considered that. "You’re getting good at saying unpleasant things gently."

"Ruth is teaching me." The doors pressed against Maren's hand, and Maren let them close.

In the lobby, the screen was still running.

Average transition cycle time down 31 percent.
Routine case handling capacity up 42 percent.
Employee satisfaction holding at 4.7.
Escalations down 18 percent.

The numbers had not changed. They would not change today, or tomorrow, or the day after, because nothing that had happened in the room upstairs had reached them yet. The review had not started. The sample had not been drawn. The board had not met. For now, the screen said exactly what it had said the morning Maren arrived, and every word of it was still true, and that was the whole problem, standing in a lobby in high resolution, telling everyone who walked past how well it was going.

She shifted her bag, and the receptionist looked up.

"Ms. Castellano. Your badge."

She had forgotten she was still wearing it. She unclipped the temporary badge and set it on the desk.

"Thank you," the receptionist said.

Her phone buzzed as she reached the doors. Gerald. Board review moved up. Thursday, 7:00 AM. A second message followed. The finding is the first item. I am walking in with the number I do not have. She read it twice, and then she pushed the door open into the cold, already thinking, without wanting to, about the instrument that did not exist yet, the one for the failure that had not happened.
